Documentation required to be uploaded in this form:

  • A copy of your most recent annual report or annual/financial statement submitted to Consumer Affairs (if applicable - organisation applicants)
  • Public Liability Insurance Certificate of Currency (to the value of $20 million (organisation applicant) 
  • If you are applying using an auspice organisation you will also need a signed certification letter from the auspice and a copy of the auspice public liability certificate of currency.
